All Region Winter Basketball Roster

In the spirit of fair play, all CYO Parish Coordinators are required to submit team rosters on behalf of their teams to the Archdiocese of NY CYO Office.  Your submission declares that you have reviewed it and that all information is accurate per CYO eligibility requirements. Rosters are due no later than November 1st or before your first scheduled game each season.

The roster must include player and guardian contact information and the following information necessary to determine player and team eligibility: player gender, birthdate, grade, school, and parishioner status. Please see the list of required fields below. 

• All rosters are considered final upon submission.

• All fields are required.

• Incomplete rosters with missing information will be returned.

Please use the link to the excel spreadsheet template below for your roster, it includes the columns required.  A google sheet may also be used, but please remember to provide access to [email protected] to view as a shared file.  Ensure all column headers are listed and information is complete for each participant.
